New/Lost Passport

NEW US PASSPORT REQUIREMENTS EFFECTIVE MARCH 8, 2005
PROCEDURE FOR THE FIRST TIME PASSPORT APPLICATION

(for individuals who have never had a passport, lost a valid passport, or whose passport was issued more than 15 years ago or prior to their 16th birthday, or whose most recent passport is damaged or mutilated)

You will need to submit the following items:

Certified birth certificate, which was issued by either state or country, along with its raised seal, or naturalization certificate, or passport issue over 15 years ago.
Two recent professional passport photos (taken no longer than 3 months).
Fill online DSP-11 and click "CREATE FORM" and print it .Please make sure that this form was filled out completely but not signed. The postmaster or clerk court house must witness your signature. Mailing address must be left blank on passport forms.

The sealed envelope is considered "expired" by the Passport Agency after 5 business days. Please please send your application to us the same day/next day it is signed.

For those who lost a valid passport, must complete lost passport form DSP-64

Check made payable to the US Passport Services in the amount of $135.00 for adult and $120.00 for children under 16.
Another $25.00 execution fee will be paid at the post office or clerk courthouse.
Proof of departure – computer generated flight itinerary from the airline, copy of airline ticket, or a corporate letter stating US departure by private aircraft (require for same/next day processing).

Present all the above mentioned items to the clerk at the authorized post office or courthouse where your signature will be witnessed. The clerk will seal the above materials in an official envelope marked TO BE OPENED BY PASSPORT OFFICE PERSONNEL ONLY. Do not write checks until directed to do so by the clerk. You must present proof of identity such as a driver’s license (which has been issued for at least 6 months). All applicants 1 year or older must appear in person at the post office or the courthouse. Minors must be accompanied by BOTH PARENTS or legal guardian with acceptable proof of ID. Send paperwork plus sealed envelope to WORLD VISA TRAVEL, INC. for processing. Do not leave documents with the clerk.
