PROCEDURE FOR PASSPORT RENEWAL 

Passport is renewable if:

• You can submit your most recent US passport.

• You are at least 16 years old when your most recent US passport was issued.

• You were issued your most recent US passport less than 15 years ago.

• You use the same name as on your most recent US passport, OR, you have had you name changed by marriage or court order and can submit proper documentation to reflect your name change.

If you answer yes to any of the above statements, then you will need to submit the following items:

• Most recent US passport.

• Two recent passport photos (taken no longer than 3 months).

• A signed authorization letter.

• Fill out DS-82 (online) (Download Form)

• Total Passport Agency Fee $135.00.

• Proof of departure – computer generated flight itinerary from the airline, copy of airline ticket, or a corporate letter stating US departure by private aircraft (require for same/next day processing).

• A check made payable to WORLD VISA TRAVEL, INC $85.00 for 3-4 business days process, $110.00 for 24 hours process.
